2/21/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in good mood evident by her facial expressions such as a smile on the face and engaging in general conversation with QMHP-C. QMHP-C informed Ashiyah that this session was going to focus on being able to comply with the rules and expectations within the home setting and communicating her thoughts and feelings when angered or not able to make positive decision making skills. 2. Behavior: Ashiyah contributed to the session and did not need any redirection throughout the session as she participated fully in session. Ashiyah discussed with QMHP-C within the session on how she gets frustrated at times because what she wants to say does not come out the way she want it to come out. She reports she struggles with being able to express her feelings and emotions. 1. Intervention: QMHP-C began the session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C redirected Ashiyah for not taking responsibility for her actions as well encouraged her to communicate effectively and decrease the use of profanity, yelling and screaming without reason, and having little to no regard for her mother. 2. Intervention: QMHP-C discussed impulse control techniques due to Ashiyah having issues being unable to complete tasks at home. QMHP-C conveyed and identified behavior expectations while suggesting that Ashiyah take control over her impulsive behaviors. QMHP-C modeled effective ways for Ashiyah to be able to identify and process behaviors. 1. Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah reported that she hate when someone else pick with her which triggers her behavior and she feels as though her mother is doing this constantly on a daily basis. Ashiyah discussed with QMHP-C that she feels as though her mother does this because she tries to hold her in the home and not allow her to go anywhere or do anything. Response: Ashiyah discussed with QMHP-C that she does complete certain task in the house but she does not complete them when her mother ask her to do them because if she is going to be in the house at all times then she has all day to complete what needs to be completed in the home. During the counseling session, Ashiyah was able to provide QMHP-C with issues that contribute to her negative behavior and what she can do to assist with those triggering behaviors.
